Use the burial fact. If you have a cemetery fact, it was probably put there by an import you may have made over the years.
As for the county twice problem. FTM doesn't require anything. I suppose you are accepting some suggestion from the fast fields feature of FTM. Fast fields are simply a typing aid. First, you don't have to accept fast field suggestions when offered. And Second, you don't have to have fast fields turned on at all. Go to Main Menu > Tools > Options > I think the warning tab - turn off fast fields. I find them helpful. If they confuse you, just turn them off, or ignore them.
If you are not talking about fast fields, please explain again in a different way what your problem is.
As far as data entry for cemeteries, folks do two modes of entry.
1) Cemetery locality in the Place field and the cemetery name in the description field of the Burial fact
2) Put the whole enchilada (cemetery name plus place)into the place field.
I prefer number two. The advantage to that is that you can go into the Places Workspace and see everyone buried in a cemetery. You can't do that if you put the cemetery in the description field.
The latest change to FTM2012 maded it possible to "resolve" Place Names that include leading text strings of cemetery names, hospital names, church names, etc, and they will go into the heirarchal view.
