One of the most important purposes of genealogy is to help understand the biological relationships. Therefore I would recommend adding the names for BOTH biological parents. If you are unsure of the missing biological parent I would place that information into a note for the missing biological parent.

If at some point in the future the biological parent is need, at least the child will have a starting point to find his biological parent.

Genetic information has always been an important factor in a person medical records, as somethings, like diabetes, have a genetic component. With the use of DNA this genetic link will be more important in the future.

It is quite easy to show both the biological releationship and the parenting relationship in FTM.

When it comes down to it the persons genetic make up is more important than the parenting relationship, when it comes to his health.
