From your descriptions, the extra note is definitely attached to something in FTM. I know that notes are attached to: Facts, Persons, Research, and Relationships. Those are listed as options in the Person Reports. Also, notes can be private. How about unchecking those options one at a time to see if the extra note goes away. Might that let you located where the GEDCOM export is picking it up from?
